Responsibilities:
- Assists the Physician Consultant and AFMH personnel with the development, implementation, and analysis of the AFMS AFMH, to include attending meetings, teleconferences, traveling to AFMH sites and providing consultative guidance and leadership to key stakeholders to include Branch members and MTF leaders.
- Provides assistance, recommendations/counseling in support of AFMS management, organizational and business improvement efforts on an as needed basis to AFMH personnel.
- Assists AFMH workgroup in writing AFI/Policies. Provides consultation on as needed to AFMH Branch, AFMOA, MTFs, and MAJCOMS. Advises the AFMH implementation team on possible AFMH model changes needed, based off of test site evaluation/observations.
- Provides consultation in the following:
- Reviews proposed and existing AFMS policies/instructions and provides subject matter recommendations to draft program implementation plans.
- Drafts program implementation plans to COR/AFMH Branch.
- Advises in the development, training/deployment of AFMH model, and program reviews.
- Advises in work process and productivity improvement, systems alignment, leadership systems improvement, organizational assessments, and assist in AFMH model implementation and developmental efforts.
- Conducts research/analysis, and makes recommendations on AFMH model implementation, development and sustainment efforts.
- Attends/supports teleconferences/meetings involving AFMH work group and implementation team functions as scheduled by COR.
- Bachelor of Science in Nursing.
- Ten (10) years’ experience as a practicing nurse in a family medicine (or other primary care) clinic.
- Five (5) years of knowledge of family medicine clinic operations, associated performance improvement programs, Patient Centered Medical Home (PCMH) program validation requirements, and consultation services in the MHS.
- Five (5) years’ experience in policy and guidance related to family medicine or other primary care service.
- Five (5) years’ experience within the AFMS. Executive-level positions held within a clinic (i.e. Chief Nurse Executive, Squadron Commander or above) or headquarter (HQ) Staff-level positions held are acceptable as direct patient care positions for this requirement.
- Five (5) years’ experience related to secure messaging, access management, telehealth, coding, disease management, case management, utilization management, referral management, and population health at a variety of levels from MTF to Air Staff.
- Two (2) years’ experience at MAJCOM or Air Staff.
